Individual and Family Services

Hiring for the following teams:

Adult Mental Health Outpatient
Working in an adult outpatient setting, Counselors provide evidenced based clinical interventions, case management support, and crisis intervention to a population that includes a significant percentage of adults who meet diagnostic criteria for severe and persistent mental illness. Seeking applicants that utilize a trauma informed lens and an awareness of cultural diversity to serve clients with a treatment team approach that includes medication management, addiction treatment, and primary care services.

Child and Family
The Child and Family Program provides services throughout the Portland metropolitan area with primary locations at each Cascadia Health Center. Child and Family services has a diverse team of qualified clinicians and case managers who provide services to youth and their families with a focus on strengthening them. Services provided are mental health individual and group counseling, crisis intervention, clinical case management and advocacy in collaboration with community partnerships. The clinical practice is respectful of culture and diversity, is family-centered and trauma-informed.

Older Adult
The Older Adult/Medically Complex Outpatient program serves older adults and those with physical disabilities, as well as behavioral health concerns. Our program has been very successful in helping individuals to improve symptoms, increase community involvement and improve quality of life. Services are provided both in-person, at our clinic or through home-visits and through phone and telehealth. Some of our clients live in long-term care facilities, others live independently or with family. Our program works as a team, with contributions from therapists, case managers, peer staff, and licensed medical providers. We also collaborate with community partners, such as facilities; Aging and Disability case managers; and medical providers.

Residential Case Management
Works in residential treatment facilities and homes with individuals experiencing serious and persistent mental illnesses, substance use disorders and other health concerns. They are responsible for writing mental health assessments and service plans, and will provide case management services, assist with intakes, discharges, and acquisition of entitlements or other benefits. The Counselor III will also provide some individual and group therapy, as well as crisis intervention. These facilities have a wide range of level of care, from semi-independent living, to secure facilities and enhanced care facilities.

Intake Team
Provides initial behavioral health assessment and treatment planning for individuals enrolling in outpatient services.

Duties:

  • Provide a wide range of therapeutic supports
  • Establish and assess level of care: complete behavioral health assessment, identify provisional diagnosis using current DSM, and develop services plans
  • Identify and assess for risk factors for harm to self and/or others in the context of the client\'s current psycho-social context
  • Provide continuity of care with other members of the treatment team and providers
  • Document and maintain clinical records and clinical progress
  • Advocate and refer to community and internal resources as necessary
Qualifications
Education: Masters Degree in a behavioral science, and eligible to register as a QMHP with MHACBO or another state licensing board. For a complete list of qualifying degrees, visit:
