Job description: The Community Services Specialist is responsible for facilitating the provision of treatment and supportive services for clients who are residential and transitioning into the community or home from residential care. The position also is responsible for providing specific services to the child\'s family. This position, Community Services Specialist is located at a Transitional Living Shelter which is operated by Youth Services Systems. This position will provide a challenging, yet highly rewarding work environment for the right candidates! This facility is a 24/7. Community Services Specialist Responsibilities:
- Prepare client for transition into their own apartment.
- Responsible for Apartment search.
- Responsible for establishing residency.
- Maintain contact as required by clients treatment plan.
- Provide supportive counseling as required by clients treatment plan.
- Responsible for written documentation of all service activities.
- Provide home visit oversight specific to gauging clients ability to adjust to the home environment.
- Daily contact for 30 days (4 face to face 3 by phone weekly)
- Provide written progress note for each service activity.
- Provide transportation as required.
- Responsible for written documentation of all services activity.
- Employees are required to use their personal vehicles in certain circumstances and as directed in the performance of their job duties.
- Other duties as assigned.
- Bachelor Degree in Human Services field preferred.
- High School Diploma or equivalent required
- Successful completion of a Background Check
- Onboarding training specific to job responsibilities
- Valid Drivers License
- Able to work afternoon and overnight shifts
