It is the overall responsibility of the Community Integration Coordinator to supervise and oversee the direct services provided to people with intellectual and developmental disabilities (IDD) in the Community Integration program. This position has many roles; the tasks that a supervisor may be asked to do include a broad range of diverse and often complex activities. These roles range from, hiring, training and supervising staff, program planning and evaluation, advocacy, working with families, and working with community members.

Work with participant and his or her teams to develop a support plan to promote the health, safety, and wellbeing of participant based on individual preferences and goals. Frontline Supervisors actively monitor, review, discuss with participant, and modify support plans to ensure most effective strategies are in place.

Operationalize participant's individual goals and identified outcomes into a coordinated support plan. Coordinate and facilitate support network meetings, maintain communication with other service providers, family, and allies, and monitor, document, and report progress toward goals.

Facilitate and support the development and maintenance of participant support networks in partnership with person supported. Support participant to explore educational, employment, volunteer, and retirement opportunities, and/or collaborate with other staff and providers to coordinate supports that will assist participant in reaching goals and actively participating in activities of his or her choice across the lifespan

Enhance professional relations among team members and their capacity to work effectively with others toward common goals by using effective communication skills, facilitating teamwork, and supporting and encouraging growth and professional development.

Coordinate and lead competency-based direct support staff training and professional development activities, including coaching and mentoring. Conduct reflective supervision and performance reviews and Responsible for all aspects of staff supervision, including scheduling and maintaining training records.

Effectively manage and oversee participant services and supports in group service settings, individual, and remote service settings, including compliance with all federal, state, and local rules and regulations, and apply ethical principles related to best practices in services and supports. Oversee the management of financial activities within scope of work assignments to ensure continued quality service delivery

Promote public relations by educating community members about the rights of people with disabilities, and advocating for and with participant for services and opportunities that promote safe, respected, and valued membership in the community.

Maintain professionalism and engage in ongoing self-development and professional development activities. Share and receive knowledge from others, support coworkers, and actively participate in the life of I-REACH 2 Inc.
Other Responsibilities
and Relationships within the Organization
Lead the Community Integration program and all related activities at the Ralph Ballard Academy.
Schedule and implement weekly program curriculum.
Prepare employee work schedules.
Determine and execute transportation for program participants.
Supervise/observe community-based community integration activities.
Collaborate with the Executive Director to engage in fundraising activities.
Under the direction of the Executive Director author and implement new policies and procedures.
Participate in Administrative On-Call Rotation.
Responsible for facilitating monthly participant meetings.
Attendance at all staff meetings.
Approve requests for leave for those in line of supervision.
Be an active contributing member of the CARF accreditation committee.
Facilitate strategic plan activities and goals, assists in quarterly newsletter production.
Completes reflective supervision and performance evaluation for those in their charge.
Complete a one service delivery observation per quarter.
Conducts ongoing going site visits/service observations.
Participates in the in-take process for individuals accepting services.

MINIMUM REQUIREMENTS:
Must possess a high school diploma/GED. Prefer Bachelor'sDegree. Must possess excellent communication skills and have the ability to create and maintain professional communication between participants, employees, stakeholders and the community.
