The employee is regularly required to speak and hear sufficiently to communicate effectively by phone or in person, at normal volumes. Specific vision is required, including adequate vision to read correspondence, computer screen, forms, etc. The employee is to have good manual dexterity.

Qualifications:



Trusted member and resident in the community being served. Strong interpersonal skills. Ability to express empathy and build supportive relationships. Counseling and health promotion skills. Ability to read, write and maintain accurate records. Knowledge of community resources including recovery, prenatal, and parenting support. Understanding of professional conduct, patient confidentiality and boundaries.

Basic Function

:


This position will assist high-risk obstetric patients, particularly those with Substance Use Disorder (SUD), by supporting care coordination across their home and community environments. The Community Health Worker (CHW), will engage in weekly home visits, help facilitate communication between patients and their care teams and provide consistent, compassionate support throughout pregnancy and the post partum period.

Help patients apply recovery focused and prenatal self-management self-management plans in the context of their home and community. Provide emotional, social, and practical support to patients navigating substance use recovery and motherhood. Connect patients with clinical, community and recovery resources. Serve as a liaison between patients and the clinical care coordination team. Provide ongoing, flexible, accessible support for patients as their needs change.

Duties and Responsibilities

:

Works under direction of the OB health care team to support pregnant and post partum individuals with SUD. Assist patients in managing chronic health conditions, maintaining medication adherence, and following treatment plans as directed by their provider. Act as a bridge between individuals, families, communities, and health and human services systems. Deliver culturally appropriate education and outreach, typically in the patient's home or community-based settings. Provide services such as guidance on accessing social services, recovery programs, emotional support, care coordination, and referrals to health screenings or behavioral health. Maintain updated client records including care plans, visit notes, and all required documentation. Educate clients and community groups on general health, wellness, parenting, and recovery topics such as nutrition, stress reduction, or infant care. Identify and engage high-risk individuals, including pregnant women, individuals in recovery, and those from underserved communities. Contact clients in person, by phone, or through written communication to encourage follow-up and completion of prenatal or treatment milestones. Distribute flyers, brochures, or educational materials to raise awareness in targeted communities. Refer community members to needed health services, including MAT (Medication-Assisted Treatment), housing programs, and parenting support. Attend community events, meetings, or health fairs to understand local issues and build relationships with community members. Collect and report data related to maternal health, recovery outcomes, and community needs to inform quality improvement efforts. * Other duties as assigned.
