GENERAL DESCRIPTION




Community Health Workers (CHWs) at CHC/SEK are passionate about serving their community. CHWs strive to understand, motivate, and help our patients achieve their goals in health and wellness by eliminating barriers, collaborating with patients to set goals, and working within a multidisciplinary care team.


This position will serve as a critical support to survivors of domestic violence, sexual assault, dating violence, stalking (DVSAS), and human trafficking (HT) by assisting them with non-clinical needs that directly impact safety, healing, and long-term recovery. The CHW will act as a culturally responsive, trauma-informed navigator--helping survivors, and any immediate, non-offending family members, who are engaging through CHC/SEK's victim services efforts and associated collaborations (e.g., Safehouse Crisis Center). This includes assisting with safety planning, emotional support, food insecurity, community referrals, benefits or assistance applications, and addressing individual barriers to excellent health, wellness, and overall safety. This role will work closely with Safehouse Crisis Center Advocates, often providing support simultaneously. It will include providing transportation in CHC/SEK vehicles, engaging in Community Response Team, and participating in targeted outreach and awareness activities within the community related to domestic & dating violence, sexual assault, stalking, and human trafficking. All services in this position are to be done in a trauma-informed, victim centered manner.


Requirements:

ESSENTIAL DUTIES



Provide services and support at multiple locations including Safehouse Crisis Center, Pittsburg State University Bryant Student Health Center, and in community or home settings. Conduct comprehensive assessments and screenings to identify health, safety, and socio-economic needs affecting individuals and families. Develop and implement individualized health and social goals, engaging patients and non-offending family members through coaching and motivational interviewing. Coordinate care and facilitate access to services, including scheduling follow-ups, assisting with prescriptions, and ensuring patients understand care plans. Connect individuals to community and organizational resources, offering clear guidance on available supports and how to access them. Provide follow-up and ongoing support through phone calls, home visits, or other contact methods to promote patient safety, engagement, and improved outcomes. Collaborate and actively participate with community partners and coalitions to address health disparities, enhance resource networks, and support coordinated community responses. Organize, facilitate, and participate in community education and outreach events addressing domestic violence, sexual assault, stalking, human trafficking, and related health topics. Assist with education, training, and victim services collaborations, including coordination with Safehouse, PSU, law enforcement, and other partners. Document all activities and support program operations, including data collection, reporting, communication between stakeholders, and the use of technology and telehealth to advance program goals.

QUALIFICATIONS



Within one year of employment, complete required training and professional development courses, including the Community Health Worker Education Program hosted by the Kansas Community Health Worker Coalition. Seeking applicants that are familiar with local community resources and health systems. Preferred applicants have education and experience in settings of Public Health, Education, Community and Family Services, Health Coaching, Case Management, Social Work, Nursing, EMT, Paramedic, etc. Bilingual preferred (Spanish, Marshallese, ASL, etc.). Valid Driver's License. Must not have more than two (2) moving violations in the past three (3) years. Basic Life Support (BLS) certification through American Heart Association or must obtain within six (6) months from date of hire. Other required training must be completed within six (6) months from date of hire and on an annual basis. Training includes, but is not limited to CTAA Pass Basic, Defensive Driving and Wheelchair Securement.

KNOWLEDGE, SKILLS AND ABILITIES



Social and cultural sensitivity appropriate to an ethnically and economically diverse patient-and employee-base. Ability to work independently, with little to no direct oversight, and as part of a multidisciplinary, collaborative team. Understand the rationale of appropriate patient care, exercising good judgment and decision-making in high-pressure, emotionally complex situations. Proficient in trauma-informed communication and active listening, especially with individuals in crisis. Effective public speaking and training skills for diverse audiences. Technology proficient, especially with Microsoft Office products, and skilled in using electronic health records (EHR), case documentation systems, and virtual care platforms (e.g., telehealth). Basic understanding of data management and analytics for grant reporting (e.g., tracking, monitoring progress, outcome evaluation). Strong organizational and time-management skills with the ability to balance multiple tasks/responsibilities to meet deadlines. Ability to adapt to frequent or sudden changes in a positive manner and be flexible with last-minute changes in schedule. Build and sustain relationships with internal and external partners across healthcare, criminal justice, and community-based systems, representing oneself in a manner that reflects the organization's mission and values. Maintain patient confidentiality and uphold ethical standards when dealing with sensitive information in accordance with organization's policy and procedure and HIPAA requirements. Communicate through appropriate channels, using proper chain of command for patient complaints or concerns.

WORKING CONDITIONS




Employee is required to regularly sit, use hands to handle/feel, reach, and communicate by talking or hearing. Frequent standing and occasional walking are required. Must regularly lift/move up to 10 pounds and up to 25 pounds occasionally. Vision requirements include close, color, and peripheral vision, depth perception, and focus adjustment. Hazardous material inventory and MSDS are available. Use personal protective equipment as required. Possible exposure to communicable diseases from patients/public. Travel is required throughout the service area. Visual acuity to read signs, watch traffic, estimate distances and color discrimination to distinguish traffic lights. Aural acuity to detect motor malfunctioning and hear traffic sounds. The employee may frequently be involved in or respond to high stress environments.
