The Partnership for Public Health Community Health Worker (CHW) plays a key role in connecting individuals and families to health and social services, building strong relationships across community partners, and empowering clients to take an active role in their health. This position supports the Winnipesaukee Mobile Health Program with a focus on improving access to care, addressing social drivers of health, and fostering long-term community resilience.
Responsibilities:
Client & Community Engagement
Connect community members with appropriate health and social services, including medical care, housing, food assistance, and financial resources, conducting in person home visits when necessary.
Conduct appropriate social need screeners with clients.
Follow up with clients to ensure they receive and utilize referred services.
Educate clients on preventive health measures, such as immunizations, screenings, and healthy lifestyle choices.
Advocate for clients within the healthcare system to ensure they receive appropriate care and services.
Demonstrate respect for clients' homes, lifestyles, and living conditions, providing support with dignity and cultural sensitivity.
Conduct office hours as necessary in community spaces to increase accessibility for clients.
Program & Data Management
Document client interactions and referrals accurately and in a timely manner in
Cerner
and other designated systems to ensure data integrity.
Cultivate and sustain relationships with community organizations serving as a connector to resources and services.
Support program efficiency by contributing to the development of standard operating procedures and guiding tools.
Use internal tracking tools to identify trends in client needs and provide feedback for program improvement.
Develop and apply public speaking and facilitation skills by presenting at community forums and leading workgroup sessions.
Share de-identified client experiences to inform and shape public health priorities.
Education
Bachelor's degree preferred, or equivalent experience in community health, human services, public health, or related fields
Certification as a Community Health Worker (CHW) preferred, or willingness to obtain certification and licensure within 6 months of hire.
Skills
Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
Strong written and verbal communication skills, including note-taking and summarizing complex information.
Excellent organizational skills with the ability to manage multiple projects and deadline.
Comfort with data collection, entry, and analysis.
Ability to work independently while also contributing to a collaborative team environment.
Strong interpersonal skills for interacting with diverse stakeholders, including community partners, health providers, and public officials.
Work Environment & Conditions:
Hybrid mix of office-based work, community meetings, and virtual coordination.
Requires occasional local travel within the region to attend meetings and events.
Flexible scheduling is required to accommodate community partner and program needs.
Physical Requirements:
Ability to work a flexible schedule, which may include night and weekends as well as being able to travel throughout the region and state as required.
Minimal requirement to reach at or above shoulder level; occasional reaching below shoulder level required.
Ability to tolerate prolonged sitting and/or standing and to lift up to approximately 25 pounds to waist high level whenever necessary.
