The Community Health Worker (CHW) provides outreach, education, care coordination, and linkage to community resources while also supporting front office functions that ensure smooth clinic operations. The CHW serves as the first point of contact for patients, assisting with check-in and check-out, connecting patients to healthcare and social services, and helping reduce barriers to care. This position blends clerical support with patient-centered engagement to improve health outcomes for vulnerable populations.
Essential Duties:
Patient Engagement & Support - 50%
1. Welcomes patients to the clinic, determine the reason for the patient's visit, initiates the needed service, and maintains orderly flow of patients through the clinic.
2. Assist patients with applications and enrollment in health and social service programs, including Medicaid, Medicare, WIC, SSI, SNAP, TANF, pharmacy assistance, housing, and transportation.
3. Support patients in navigating the healthcare system by scheduling follow-ups, coordinating referrals, and providing linage to specialty and support services.
4. Conduct follow-up contacts to monitor progress, encourage treatment adherence, and address barriers to care.
5. Participate in community outreach events and health fairs to promote clinic services
Front Office & Care Coordination - 50%
6. Perform check-in and check-out operations, including registration, updating demographics, scanning documents, verifying insurance, and collecting payments.
7. Answer phones, schedule appointments, and provide appointment reminders.
8. Route clinical calls appropriately and provide general clinic information.
9. Maintain an organized front office environment and ensure availability of required forms.
10. Assist with Patient-Centered Medical Home (PCMH) duties including scheduling same-day appointments, collecting patient feedback, and supporting access needs.
11. Document patient encounters and CHW services in the electronic health record (EHR) in compliance with agency and Medicaid standards.
Additional Duties:
12. Provide transportation assistance for patients, including arranging services through Foothills Express, BMT, and case management programs; may provide direct transport as assigned.
13. Collaborate with case managers, peer support specialists, and clinical staff to ensure holistic care coordination.
14. Assist Project Director with monthly reminder calls and mailings of Advisory Board information.
15. Performs all other duties, as assigned by the assigned supervisor.
Education, Experience & Training:
High School Diploma and/or GED required. At least 2-3 years of experience in health care setting required. CHW certification (or willingness to obtain certification through the Kentucky Office of Community Health Workers within 12 months of hire) preferred. Certification in CPR is preferred.

Position requires occasional or frequent moderate physical activity. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand; walk, climb, balance, stoop, kneel, crouch, crawl, and use hands to manipulate, handle or feel; and reach with hands and arms. The employee is occasionally required to lift/and or exert force up to 50 pounds.
Other Requirements:
Must have a valid driver's license and the flexibility and willingness to change schedules and duties as needed according to staff availability. Must be willing to travel the service area as needed. Travel is primarily local or within the service area during the business day, although some out-of-area and overnight travel may be expected. Agency policy regarding drug and alcohol testing and criminal background check applies to this position. Must maintain a high level of professionalism, demonstrated by a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency.
