Position Summary:




The Clinical Team Manager leads an Interdisciplinary Team of Nurses, Social Workers, and Chaplains in providing excellent, patient- and family-centered end-of-life care. The Clinical Team Manager has the responsibility and authority for planning, coordinating, implementing, and evaluating patient care services to ensure that the provision of care meets established standards of practice, regulatory requirements, and agency policies and procedures. The Clinical Team Manager is responsible for managing the day-to-day clinical and facility operations of the Casey House Team, including oversight of staffing, workflow, safety, and coordination of facility resources to support quality care delivery and regulatory compliance.

Minimum Qualifications:



Education: Bachelor's degree in Nursing from an accredited college or university OR equivalent experience. Licensure/Certification: Current, active, and in goodstanding nursing License from the Maryland Board of Nursing or a compact state. CHPN is required within 1 year of start date. Experience: Minimum of 2 to 3 years of progressively responsible hospice experience preferred. Management experience preferred Additional Skills: Good knowledge of hospice. Demonstrated knowledge of state, federal and regulatory standards as applied to hospice organizations. Familiarity with interpersonal dynamics and ability to maintain effectiveness in an emotionally stressful environment. Demonstrated leadership ability, excellent communication and interpersonal skills. Demonstrated critical thinking skills and ability to exercise good judgment. Ability to coordinate interdisciplinary services. Demonstrated clinical expertise within respective discipline. Demonstrated computer skills.

Duties and Responsibilities:



Lead and supervise an interdisciplinary team (Nurses, Social Workers, Chaplains). Collaborate with disciplinespecific clinical leaders. Conduct joint visits and provide daily oversight to ensure care standards. Perform competency reviews (initial, annual, periodic). Participate in hiring processes and make recommendations. Assign and adjust staff schedules for optimal coverage. Conduct annual evaluations and support goal setting for clinicians. Ensure care delivery aligns with regulatory standards and agency policies. Monitor documentation and conduct chart reviews. Report quality metrics to Quality team. Observe staff adherence to CDC infection control guidelines. Facilitate and support interdisciplinary team (IDT) meetings. Promote collaboration and conflict resolution among team members. Ensure accurate documentation of IDT outcomes. Promote excellent customer service and proactive issue resolution. Maintain effective communication with other teams and departments. Keep leadership informed of operational issues. Support inservice and continuing education programs. Coach staff on professional standards and best practices. Participate in oncall clinical administration. Provide backup support to other Clinical Team Managers.
