TITLE: Clinical Staff Pharmacist

LOCATION: Sundance, WY

What they are looking for:

• BS or PharmD required.

• Wyoming state licensure or eligible.

• Position is day shift. Limited weekends and rotating holidays

• The range is $114K-$156K depending on experience plus shift differentials and a full benefit package including relo/sign-on bonus.

• Serves as clinical pharmacist providing care for Long-Term Care, Inpatient, Outpatient pharmacy services. Assess and review patients medical statues and advises on prescribed medication to optimize patient outcomes.

• Provides medication therapy management services in collaboration with other providers to assure that medication therapy is safe, effective, efficient, and cost-effective.

• Pharmacy service provided include interdisciplinary patient care, medication reconciliation, medication therapy monitoring, long-term care consultation, medication orders management, patient education, drug information, drug dosing, and education activities.

• Provides oversite of pharmacy technicians.

• Will provide clinical and distributive tasks to ensure care to the patients.

• The facility is located south of Sundance and is a critical access hospital that also has a 58 bed nursing home.

• The position reports to the Director.
