Job Description

The PACE Your LIFE Clinical Operations Manager plays a pivotal leadership role in a PACE interdisciplinary team, overseeing the delivery of high-quality, participant-centered nursing care across clinic, home, and community settings. This position ensures compliance with federal and state regulations, supports team members development, and fosters a culture of safety, dignity, and excellence in geriatric care. This position works closely with the PACE Your LIFE Medical Director overseeing clinical and medical team members. This includes registered nurses, licensed practical nurses, medical appointment schedulers, medical assistances, certified nursing assistants and patient care assistants in the clinic, day center, and community.

Essential Duties and Responsibilities:



1.

Clinical Operations & Oversight




a. Manage the daily operations of the PACE clinic, ensuring efficient workflows and timely care delivery.


b. Supervise the nursing team, medical assistants, and other clinical personnel.


c. Coordinate participant care across settings (clinic, home, hospital, long-term care).


d. Monitor clinical documentation for accuracy, timeliness, and regulatory compliance.


e. Ensure compliance with policies and procedures in conjunction with the Center Director. Makes recommendations for policy and procedure changes based on operational issues.


f. Ensure the adequacy of equipment and supplies for both the clinic and homes.


g. Conduct initial, semi-annual, and annual comprehensive nursing assessments (as needed).


h. Provide appropriate clinical care and services within your scope of practice


i. Collaborate with pharmacy services to ensure participants' medications are accurate and available.


j. Provide some on call coverage.

2.

Team Leadership and Development




a. Recruit, onboard, and train nursing and clinic team members.


b. Conduct performance evaluations and support professional growth.


c. Lead regular team huddles and participants in IDT meetings.


d. Ensure team members adherence to infection control, safety, and ethical care standards.

3.

Compliance and Quality Assurance




a. Maintain compliance with CMS, state licensing, and PACE-specific regulations.


b. Lead audits, incident reviews, and corrective action planning (CAP).


c. Track and report quality indicators (e.g., falls, hospitalizations, medication errors).


d. Collaborate with compliance and quality teams to implement best practices.


4. Participant-Centered Care


a. Advocate for participant dignity, autonomy, and safety.


b. Support care planning and transitions of care.


c. Address participant and family concerns with empathy and professionalism.


d. Ensure culturally competent and trauma-informed care delivery.

Qualifications:



1.

Education:




a. High School Diploma or GED required.


b. BSN with current Delaware licensure.

2.

Credentials/Licensure Required:




a. Must possess a current Driver's License and maintain acceptable driving record.


b. BLS for healthcare providers or CPR Certification.

3.

Experience:




a. Supervisory and administrative experience is required.


b. One year of experience working with older adults.


c. Proficiency in Word, Excel, Outlook, and PowerPoint is strongly preferred.


d. Must be proficient with HER systems.

4.

Knowledge Skills, and Other Requirements:




a. Basic understanding of automation and information technology management systems.


b. Ability to coordinate and collaborate with others in a team environment.


c. Ability to act as an advocate for the frail elderly.


d. Ability to provide and encourage excellent customer service to participants, caregiver, team members, and vendors.


e. Freedom from illegal use of drugs, and freedom from use and effects of use of drugs and alcohol in the workplace.


f. Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a healthcare-related setting are ineligible for employment in this position.
