This position requires professionalism, compassion, accuracy, and follow-through. Intake is not clerical--it is strategic. When done right, it protects compliance, drives revenue, and builds trust from day one.
Responsibilities
Client Intake & Initial Screening
Serve as the primary point of contact for prospective clients, families, and referral partners
Documentation & Compliance
Accurately enter and maintain client information in the agency's intake and scheduling systems
Collect required intake documentation, consents, and service agreements
Ensure all intake records align with agency policies and applicable regulations
Maintain strict confidentiality and HIPAA compliance at all times
Care Coordination Support
Schedule in-home consultations, assessments, or care start dates
Communicate intake details to Care Coordinators and Supervisory staff
Track pending intakes and follow up to completion
Support smooth handoff from intake to active service delivery
Client Experience & Relationship Building
Provide a warm, respectful, and confidence-building experience for clients and families
Address initial questions, concerns, and expectations with empathy and clarity
Represent Nancy's Home Care's mission, values, and standards of excellence consistently
Administrative & Growth Support
Track intake activity, lead sources, and conversion outcomes as assigned
Support referral tracking and reporting
Assist with onboarding process improvements and growth initiatives
Answer and return phone calls, emails, and online inquiries promptly and professionally
Conduct structured intake interviews to assess care needs, service eligibility, and service level
Explain Nancy's Home Care services, rates, minimum hour requirements, and care process clearly
Determine next steps and readiness for care initiation
QualificationsRequired
High school diploma or equivalent
Minimum of 1-2 years of experience in client intake, customer service, healthcare, or human services
Excellent verbal and written communication skills
Strong organizational skills and attention to detail
Comfort with phones, data entry, and basic computer systems
Preferred
Experience in non-medical home care, healthcare, or social services
Familiarity with intake systems, EMRs, or scheduling software
Understanding of personal care, companion care, and homemaker services
