ABOUT MEALS ON WHEELS
Our mission is to provide nutritious meals, wellness checks, and caring contact to elderly, disabled homebound, and others in Tulsa and surrounding areas.
JOB SUMMARY
The Client Care Coordinator plays a key role in our More Than a Meal program, serving as a lifeline for seniors and individuals with disabilities who need support to stay healthy and independent at home.
This role combines care coordination, resource navigation, client advocacy, and relationship building. You will assess needs, develop Service Plans, address social determinants of health, connect people to community resources, and provide meaningful support that truly changes lives.
ESSENTIAL DUTIES & RESPONSIBLITIES
Provide comprehensive care coordination including assessing needs, determining eligibility, responding to changes in condition, making referrals, tracking progress, and connecting clients to community resources.
Conduct telephone and in-home visits as part of ongoing service monitoring.
Build and maintain strong relationships with clients, caregivers, and community partners.
Maintain accurate and confidential documentation in database systems, mobile applications, and web-based tools.
Assist with meal delivery and wellness checks only when volunteer coverage is unavailable.
Support organizational goals by improving communication, efficiency, and mission impact.
Perform other duties as needed to support the mission of Meals on Wheels of Metro Tulsa.
MINIMUM REQUIREMENTS
Nonprofit business experience is preferred.
Bachelor's degree preferred.
Minimum two years experience in care coordination and or case management.
Minimum two years experience working with seniors or individuals with disabilities.
SKILLS & ABILITIES
Excellent oral and written communication skills.
Ability to interact professionally, compassionately, and respectfully with diverse populations.
Strong technology skills and proficiency in MS Office and Office 365.
Goal oriented, mission focused, and highly organized.
Experience training or supervising others is a plus.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
This position is office-based. It is a full-time, non-exempt position with typical hours Monday-Friday 8:00-4:00. This position involves frequent sitting and occasional standing, walking, bending, stooping, and squatting. The employee is frequently required to use hands and fingers, handle or feel; and reach with hands and arms. The role routinely uses standard office equipment such as computers, phones, copiers, and filing cabinets. While performing the duties of this job, the employee is regularly required to see, talk, and hear.
