The Client Care Coordinator plays a vital role in ensuring the overall well-being, engagement, and care coordination of residents in the Mental Health Residential Program. This position serves as a bridge between residents, clinical staff, families, and external providers--promoting recovery, stability, and a supportive therapeutic environment. The Coordinator facilitates communication, assists with care planning, and ensures residents receive high-quality, person-centered services consistent with their individualized treatment plans.
Resident Support & Case Coordination
Serve as the primary point of contact for residents regarding daily needs, appointments, and care coordination.
Assist residents in understanding and following their individualized treatment and recovery plans.
Coordinate intakes, assessments, discharges, and transitions between levels of care.
Monitor residents' progress and communicate updates to the clinical team.
Ensure client records, consents, and documentation are accurate and up to date.
Education & Experience:
Bachelor's degree in Psychology, Social Work, Counseling, or related field required.
Minimum 1-3 years of experience in mental health, social services, or residential care settings.
Job Type: Full-time
Pay: $22.00 - $26.00 per hour
