Position: Client Care Coordinator

Reports to:
President of CCA Home Care
The Client Care Coordinator is the face of CCA Home Care--building strong referral relationships and working closely with families to guide them through the onboarding process into home care services.
What You'll Do
Build and maintain strong, in-person relationships with referral partners across Omaha
Meet with families to understand their needs and onboard them to CCA Home Care services
Serve as a compassionate, trusted resource for clients and their loved ones
Represent CCA Home Care at community events, networking groups, and partner meetings
Collaborate with our administrative and caregiver teams to ensure smooth service delivery
Maintain accurate documentation and coordinate with the President and office staff
Positively promote CCA Home Care's values of dignity, compassion, and high-quality support
Who We're Looking For
The ideal candidate will have:
Strong relationships & credibility within Omaha's eldercare community
(Assisted living facilities, case managers, hospital discharge planners, senior centers, etc.)
A compassionate, family-centered approach
Able to sit with families, listen deeply, and guide them through care decisions with empathy.
Excellent communication & organizational skills
Professional, polished, and able to handle multiple responsibilities with confidence.
A self-starter mentality
Comfortable working both independently and collaboratively.
Valid driver's license and reliable transportation
