All of Us In Recovery (A.I.R.) provides person-centered, comprehensive case management services to help participants access essential community resources. These include behavioral healthcare, peer support, housing, medical care, advocacy, education, employment, community and social supports, and assistance navigating the criminal justice system.
Position Summary:
Care Coordinators support clients in accessing and navigating community resources to enhance well-being and achieve recovery goals. This role supports participants in Free Through Recovery and Community Connect, with potential opportunities in the 1915(i) Medicaid program.
Responsibilities include:
Assessing client needs and developing personalized care plans.
Connecting clients to services such as healthcare, housing, employment, and social supports.
Collaborating with service providers and tracking client progress.
Providing ongoing support to promote successful community reintegration and reduce recidivism.
Required Qualifications:
Bachelor's degree in social work or a related field,
OR
1 year of case management experience and/or experience in the field of human behavioral health
Valid driver's license
Basic data entry and documentation skills
Goal-oriented, self-driven, with a passion for advocacy
Strong communication and problem-solving skills
Preferred Qualifications:
Completion of ND State-required training
Experience working within the criminal justice system, including knowledge of Parole and Probation processes
Compensation:
Commission-based pay, dependent on clients worked with
Part-time hours with potential to transition to full-time as caseload grows
