case manager/community-based rehabilitation specialist (CM/CBRS) to
work with primarily with youth and teens
of different socioeconomic and cultural backgrounds in and around the Boise/Meridian/Nampa areas. This
position has the potential to build to full-time
in the future.
CBRS
providers assist clients who have difficulty functioning in their home and community settings. They help individuals gain the capacity for independence by teaching them skills on a continuous or intermittent basis in areas such as: vocational/educational, financial, social relationships/support, family, basic living skills, housing, community/legal, or health/medical.
CM
provide clients with support while they develop the skills needed to become independent and successful. CMs help clients by linking them to financial help, housing, vocational, academic, medical or legal assistance, or any other resource to meet their family's basic needs. CMs also help by coordinating and advocating on the client's behalf and by navigating a variety of systems and processes.
Some
responsibilities
include
, but are not limited to the following:
Assess client needs.
Assist the client with identifying and setting goals.
Connecting/linking client to needed services.
Advocating on behalf of client.
Planning and implementing learning activities.
Teaching clients skills needed to function and gain independence.
Use a variety of teaching modalities, such coaching, modeling, traditional learning instruction, etc.
Coordinate with other providers, agencies, and programs involved in the client's care to ensure optimal treatment.
Write progress notes, create and update client treatment plans, and conduct 90 day case reviews.
Qualifications:
Must have a
bachelor's degree
Must have a valid
driver's license and registered vehicle with adequate insurance
.
Evenings (after-school hours) and weekends are a MUST.
Be able to
communicate
effectively, advocate, conduct research, and be resourceful.
Be caring, outgoing, and empathetic.
Be able to meet deadlines.
At times, be willing to work with interpreters.
