Program Overview


YAS provides not only clinical treatment but also continuity of care with community-based support services including case management, vocational, educational and recreational rehabilitation, housing and psychiatric services. The underlying philosophy is to promote people recovery, independence and empowerment in a community setting.

Program Services & Goals



Clinical diagnostic assessments Assistance in developing social system supports Fostering school success and vocational readiness Fostering independent living, pro-social behaviors and social skills Fostering relationships by using both traditional and non-traditional supports Emphasizing stabilization through supports rather than office-based therapy Reinforcing substance misuse prevention Using planned step-downs to less intensive levels of support An integral part of the program is supported housing, which includes transitional housing and drop-in center for people living on their own or with family.

Case Manager position


Provide and coordinate supportive services for clients with serious and prolonged mental illness.

Position Overview


As a case manager you have a unique opportunity to forge a therapeutic connection with an adult who has been diagnosed with a mental health disorder. When individuals are asked what made the difference in their lives, the most frequent response is that they had someone they worked with who gave them hope that they can have what everyone wants: safe and decent housing, access to quality mental health and medical care, meaningful personal relationships, productive activities including work, and a connection to the community in which they live.

This is a job in which every day is different, where you have the independence and flexibility to structure your time but the support and back up of seasoned and professional supervisors as well as the opportunity to collaborate with members of our clinical and community support teams.

This position involves extensive work in community. Use of agency vehicles as well as use of own car (mileage reimbursement).



Provide services and conducting daily business and interactions with clients, staff outside contacts and referral sources with positive and professional customer relations. Provide assessment of assigned clients community services needs. Develop, monitor, and update the Community Service Plan with the client. Develop and maintaining required relations with staff and other service programs for the purpose of obtaining services. Initiate client involvement in the agreed upon services, including monitoring and follow through to assure appropriateness and continuity of the services. Provide individual or group counseling, education, support, transportation and daily living skill development services to assigned clients to enhance client's independent functioning. Provide advocacy and ensuring continuity of care for assigned clients. Provide accurate, appropriate and complete records, documentation and reports. Perform other duties as assigned.

Schedule



Monday through Thursday 9:00 A.M.. to 5:30 P.M. and Friday 8:30 A.M. - 5:00 P.M.

Qualifications


Bachelor's Degree in relevant human services field or Associate's Degree in relevant human services field plus two years of relevant experience.

Salary Range


$19.00 - $21.00 per hour depending on experience

Bilingual Spanish/English Fluency


$19.48 - $22.04 per hour depending on experience
