Function/Scope:




The Supportive Services for Veteran Families (SSVF) Case Manager is responsible for providing support and services to participating veteran households. The Case Manager may assist in connecting the Volunteers of America Pennsylvania (VOAPA) SSVF program with local and professional agencies and staff to help generate community partnerships, client referrals, and community resources. This position assists with ensuring that the SSVF program operates and delivers services according to program guidelines established by the U.S. Department of Veterans Affairs (VA), as well as VA grant requirements, VOAPA policies and procedures, and federal, state, and local regulations.

Qualifications:




The candidate must have a bachelor's degree related to human services-related field or at least two years of experience in case management and counseling (preferably with veterans and/or homeless individuals and families). In addition, the candidate must have strong oral and written communication skills; proven organizational and time management skills; keen attention to detail; the ability to develop effective working relationships with clients, staff, and community partners; maintain a professional appearance; and have a sensitivity to vulnerable populations.



This position requires driving a company and/or a personal vehicle. The successful candidate must have a driver's license with the State of Pennsylvania, possess vehicle liability coverage, and have maintained a good driving record. This also requires frequent travel throughout the program's multi-county serving area.

A. Principle Duties:




1. Provide comprehensive case management services for clients in accordance with policies and procedures and contract/funding source requirements.


2. Maintain a caseload of active clients, consistent with monthly program goals.


3. Develop, establish, and maintain effective outreach services and positive working relationships with SSVF staff and local/professional community agencies and resources.


4. Perform weekly outreach at designated homeless shelters, screening both referred and walk-in potential clients for eligibility and intake.


5. Maintain consistent contact with clients in accordance with program and agency regulations, to include phone contact, email, in-home visitation, community visitation, and in-office visitation.


6. Perform basic counseling skills (e.g., motivational interviewing, strengths-based approach) while employing best practices.


7. Assist clients with achieving personal housing goals in accordance with an Individualized Housing Plan.


8. Update the Individualized Housing Plan monthly (or sooner) to reflect changes in client circumstances or goals.


9. Maintain client confidentiality at all times, using best practices and adhering to local, state, and federal guidelines.


10. Work collaboratively with community housing providers, veterans service providers, homelessness service providers, the VA, family service providers, employers, and other professionals to provide housing stability, and where needed, income stability.


11. Ensure that clients' and program records are generated and maintained in accordance with policies and procedures and contractual and regulatory requirements.


12. Compose accurate, timely case notes on each client interaction using established templates and procedures, and submit weekly to the Senior Case Manager for review and correction.


13. Manage continuous improvement of the case management process.


14. Other duties as assigned.

B. Operational Functions:




1. Act as a liaison between the program and funding/regulatory and government agencies.


2. Assist in developing, implementing, and maintaining program operating documents and forms as requested and/or required.


3. Develop, maintain, and/or manage reports, including client file notes, as required. Collect, maintain, and disseminate data on clients, services, and operations as required.


4. Ensure all applicable federal, state, and local government and contractual regulations and requirements are met or exceeded.


5. Maintain compliance with VOAPA policies and procedures.


6. Ensure that all duties are carried out in a timely and orderly fashion so that SSVF functions in the most efficient manner possible.

C. Professional Development:




1. Routinely meet with supervisory staff for individual, face-to-face supervision to discuss issues related to job performance or implementation.


2. Participate in staff meetings in accordance with program and agency requirements.


3. Participate in staff training in accordance with program and agency requirements.

VOAPA's SSVF staff work in a non-stationary environment and frequently travel throughout the programs regional serving area. They also engage with a diverse public and may be subjected to interruptions throughout the day. At times, the staff may be required may be required to assist veterans who are in distressing circumstances and environments.
