About the Role: Integrated Care




Integrated Care recognizes that behavioral health is a core component of a person's overall well-being- "mind the body, mind the mind." This position supports the delivery of behavioral health services within primary care settings, increasing access and reducing barriers by bringing care directly to an individual's medical home (primary care facility).


Team members provide a range of services, including behavioral health screenings, assessments, case management, and individual or group therapy. Services are offered in fast-paced primary care clinics where individuals often present with both physical and behavioral health needs, requiring a flexible, collaborative, and patient-centered approach.


This position supports both scheduled and walk-in clientele, balancing brief interventions with more structured therapy. Staff may work in one or more of the seven integrated Sunrise locations and must be comfortable moving between sites as program needs evolve. Staff must maintain accurate and timely documentation across multiple electronic health records (EHR) systems while collaborating with multidisciplinary medical teams to coordinate comprehensive, whole-person care.




Key Responsibilities:



Provide behavioral health services to clients, including brief interventions, case management, and support toward treatment goals. Collaborate with therapists, medical staff, and other professionals to coordinate care and prevent crises or hospitalization. Connect clients to community resources such as medical care, housing, vocational services, and social supports, assisting with applications and documentation as needed. Build strong relationships with community agencies and other partners to support client needs. Engage clients in education sessions, classes, or groups, adapting teaching techniques to support learning and participation. Coach clients in daily life skills, including self-care, managing finances, coping strategies, and problem-solving. Support families, caregivers, and other providers to remove barriers and strengthen client outcomes. Adapt services to meet client needs, including outreach, accompanying clients to appointments, and helping with medication. Monitor and document client progress using electronic systems, ensuring accurate and timely records for treatment and program requirements. Support agency and payor requirements, including billing, authorizations, and ethical coding. Participate in assessments, treatment planning, and interventions for behavioral health needs, including Integrated Care and specialized populations. Advocate for clients with schools, courts, hospitals, and other agencies to ensure access to services and smooth transitions in care. Provide intake, translation, or other program-specific support as needed.

Qualifications:



A bachelor's degree in a human services field from an accredited college or university is

required.

A Valid Driver's License and driving record that meets agency insurability requirements is

required.

Bilingual Skills (English/Spanish) highly desired. Successful completion of background, fingerprinting, and drug testing



Experience:




No prior experience is required, though it's preferred.
