Thomas Allen\'s Case Management Division is Hiring! $1,500 HIRING BONUS LOCATION: Office in Edina with travel for visits with individuals served in the Twin Cities Metro. This position is in-person. Hybrid work-from-home opportunities are available after the first 3 months of employment! HOURS AVAILABLE: Monday - Friday, flexible 8am - 4:30pm. We\'re hiring for one person in this division! JOB SUMMARY: As a Case Manager with our long-term care (LTC) Hennepin County division, you will conduct assessments, create and review care plans, and coordinate services with individuals served and the interdisciplinary team. You will also connect individuals served with community resources and ensure waiver coordination and documents meet lead agency guidelines. Our LTC Hennepin County team is full of supportive and generous co-workers who are committed to providing quality services to those we serve. REQUIREMENTS/MUST HAVE: Bachelor\'s degree in social work and LSW, or a bachelor\'s degree in a related field such as psychology, human services, or sociology, or Registered Nurse, or Public Health Nurse Background clearance Ability to effectively communicate in English, written and verbally Driver\'s license, reliable vehicle you can use for work purposes, acceptable driving record, and proof of auto insurance Must be at least 18 years of age NICE TO HAVE: Experience in CADI, BI, EW, CDCS, and/or AC Knowledge of MA, Medicare, and MN Healthcare programs Knowledge of community resources and providers Great organizational skills and attention to detail Knowledge of computer basics and familiarity with Microsoft Office suite Bi-lingual oral and written skills in Spanish, Hmong, Somali To view other open positions, click here.
