Thomas Allen\'s Case Management Division is Hiring! $1,500 HIRING BONUS LOCATION: Office in Edina with frequent travel for visits with individuals served in the Twin Cities Metro. This position is in-person, with telecommuter options available to highly qualified candidates. Hybrid work-from-home opportunities are available after the first 3 months of employment! HOURS AVAILABLE: Monday - Friday, flexible 8am - 4:30pm. Part time scheduling available to qualified candidates. JOB SUMMARY: As a Case Manager with our Hennepin County DD Unit, you will conduct quality visits, create and review care plans, and coordinate services with individuals served and the interdisciplinary team. You will also connect persons served with community resources and ensure waiver coordination and documentation meet lead agency guidelines. Our Hennepin DD team is full of supportive and person-centered colleagues who are committed to providing quality services to the people we serve. REQUIREMENTS/MUST HAVE: Bachelor\'s degree in social work and LSW, or a bachelor\'s degree in a related field such as psychology, human services, or sociology, or Registered Nurse, or Public Health Nurse Must have at least one year of full-time experience working with people with developmental disabilities. Background clearance Ability to effectively communicate in English, written and verbally Driver\'s license, reliable vehicle you can use for work purposes, acceptable driving record, and proof of auto insurance Must be at least 18 years of age NICE TO HAVE: Experience with Rule 185 and DD Waiver Knowledge of MA, Medicare, and MN Healthcare programs Knowledge of community resources and providers Great organizational skills and attention to detail Knowledge of computer basics and familiarity with Microsoft Office suite A language stipend is available to multi-lingual candidates to meet community needs.
