What You'll Do
As a Case Manager, you'll help individuals access and maintain the services they need to thrive. You'll provide:
Holistic assessments and person-centered service planning
Coordination of medical, social, educational, and community supports
Crisis intervention and emergency on-call support
Ongoing monitoring, reassessment, and advocacy
Collaboration with families, schools, jails, and partner agencies
Support for individuals in the least restrictive, most empowering environments
You'll also contribute to group therapy, team meetings, and continuous quality improvement efforts--all while building strong relationships and promoting recovery-focused care.
What You Bring
Bachelor's degree in Social Work, Education, Nursing, or related field
State registration as an RN or LBSW (or eligibility to obtain)
At least 1 year of relevant experience in mental health or nursing
Strong communication skills and functional computer literacy
Valid Michigan driver's license and acceptable driving record
Lived experience with mental illness, developmental disabilities, or substance use is valued
