Care Navigator
Job Description
: This position provides operational support and transition duties, as assigned, for care management services for identified highest risk (complex)SelectHealth members by working collaboratively with the care management team, patients, family caregivers, significant others, healthcare providers, payers, community-based providers, and other involved parties to provide effective, efficient, and patient-centered care management services. Position Details: This role will work Monday-Friday from 8:30am-5:00pm. This position is primarily remote, but will require in office presence at the Murray SelectHealth location on a monthly basis. Minimum Qualifications:
- One year of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination

- Demonstrated communication skills
- Knowledge of medical issues
- Ability to represent the department with other agencies
- Demonstrated problem solving skills
- Must have excellent interpersonal and communication skills.
- Ability to adapt quickly as needs arise.
- Knowledge of available health resources.
- E xperience working with social determinant of health needs for our Medicare/Medicaid population.
- Bilingual English/Spanish.
- Discharge/transition planning, healthcare coach, health advocate, or medical assistant experience.
