Care Manager - Crisis Stabilization
Seeking an organized and independent thinker who takes initiative to meet each client's individualized and unique needs. The Care Manager delivers care management services to a caseload of clients currently being helped at the Adult Crisis Stabilization program, a short-term hospital diversion program that supports adults experiencing a mental health crisis.
Adult Crisis Stabilization helps prevent emergency room visits and assists people as they step down from inpatient hospitalizations. Last year we served 141 people who otherwise would have had to visit the ER for mental health reasons. Working closely with the new Mental health Urgent Care and other community supports, Adult Crisis Stabilization is a critical part of Vermont's system of care.

Responsibilities:
Provides case management to adults with a variety of diagnoses
Provides direct services and support to clients; determines quality interventions, maximizing resources and fostering the clients' independence
Completes referrals to other services, community resources and different levels of care
Establishes relationships and partnerships with local resources
Assist clients with crisis prevention, de-escalation and conflict management
Establish professional relationships with clients and families assigned to caseload
Provides brief treatment (crisis stabilization, supportive counseling, skill building)
Requirements:
One year of experience working in Human Services, particularly in mental health, Social Work, Psychology, substance use, or developmental disability field
Knowledge of community resources, public benefits, diverse populations, crisis intervention/de-escalation, and/or lived experience
Driver's license and use of a personal vehicle

Starting at $23.22/ hour
