A Clinic Care Coordinator RN works with primary practice team members to meet quality program requirements, deadlines, and deliverables while identifying and meeting patient care coordination needs.
Job Duties:
Assist patients in navigating the health care system as a patient advocate.
Assist in managing high-priority patient care plans and coordinating patient care needs.
Assist in the development and coordination of primary care practice team project improvement work.
Assist in the development and coordination of primary care practice team project improvement work.
Coordinate continuity of care with patients and families following Emergency Department and Hospital Inpatient discharges.
Develop and implement methods of pre-visit planning working with the primary care provider(s) and team(s) to prepare for patient visits.
Demonstrate a proficient level and high-quality nursing practice to provide safe, effective, and holistic family-centered care for all patients and families of the clinic
Monitor quality metrics and actively work to assist patients and families in meeting quality measure outcomes.
Maintain annual unit specific competencies and education fair.
Adhere to White River Health System's Standard of Behaviors.
Qualifications:
Current RN license by the Arkansas State Board of Nursing
Current BLS Certification
Basic knowledge of Microsoft Office Software
Minimum of 2 years' experience in case management and/or quality improvement work in primary care-preferred

1. Must be able to report to work fit for duty and free of any adverse effects of illegal drugs, medical marijuana, prescription medication, and/or alcohol.
2. Must be able to effectively communicate both orally and in writing with other individuals.
