What You'll Do
As a Care Coordinator, you'll play a critical role in ensuring seamless communication, flawless organization, and consistent high-quality care for our clients and caregivers.
Your key responsibilities include:
Client & Caregiver Support
Serve as a trusted, professional point of contact for caregivers, clients, and families
Personally welcome new clients with a care call and home visit
Handle concerns with poise, professionalism, and empathy
Maintain proactive communication through regular service check-ins
Scheduling with Purpose
Manage and coordinate schedules with accuracy and foresight
Balance client needs and caregiver capacity to deliver exceptional care
Training & Onboarding
Lead warm, professional onboarding experiences for new caregivers
Organize and track coaching and training opportunities within care teams
Field Visits & Quality Checks
Conduct in-home visits, care assessments, and quality assurance checks across Northern Michigan
Ensure care delivery aligns with client expectations and organizational standards
What We're Looking For
Proven background in office administration (Healthcare, senior care, medical office, or service coordination experience highly valued)
Exceptional communicator--professional, empathetic, and clear in all interactions
Highly organized and detail-oriented--able to manage multiple priorities with precision
Strong problem-solving and leadership skills--proactive in identifying needs and creating solutions
Proficiency with Microsoft 365 (Outlook, Teams, Word, Excel) and adaptability with new software
Valid driver's license, insured vehicle, and ability to pass a background check
Bachelor's degree preferred, or equivalent professional experience
