is seeking a Targeted Case Manager/ Care Coordinator (Contracted) at their Oakland Maine Children's Services program. Full Time, flexible to Contracted.

Requirements of the Targeted Case Manager:

Bachelors degree in any field 1+ year in Children's Services or 2 + years in social services preferred Case Management experience is preferred Exceptional communication skills Strong organizational and writing skills and ability to multitask, prioritize, and problem solve in a fast-paced environment. Microsoft Office Suite Monday - Friday 8 hours a day in Oakland, Maine

Responsibilities of the Targeted Case Manager

The Case Manager will manage a case load of 15 - 25 clients. Helping to manage their appointments Education Support Transportation needs Coordinate and facilitate team meetings. Write comprehensive assessments, quarterly reviews and continued stay reviews Attend home visits, doctor appointments and family team meetings Research, coordinate and communicate with team members
