Position Summary

The Care Coordinator is responsible for managing client and caregiver supervisory visits, ensuring that every client receives the right care at the right time. This role requires excellent communication, strong problem-solving skills, and the ability to work in a fast-paced environment.

Key Responsibilities

Coordinate and maintain client care plans to meet client needs. Serve as the primary point of contact for clients, families, and caregivers regarding scheduling and service updates. Respond promptly to changes and updates regarding clients Assist with caregiver recruitment, onboarding, and orientation when needed. Support caregivers by providing clear instructions and follow-up on client concerns. Communicate effectively with office staff to ensure continuity of care and high service standards.

Qualifications

Previous experience in home care, healthcare scheduling, or customer service preferred. Strong organizational skills with attention to detail. Excellent verbal and written communication abilities. Ability to handle multiple tasks and stay calm under pressure. Proficiency with scheduling software and Microsoft Office Suite. Compassionate and professional demeanor when working with clients and families.

Schedule & Compensation

Full-time occasional on-call rotation may be required. Competitive hourly wage or salary based on experience.
