Care Coordination Support Specialist

to join our dynamic Care Coordination team, where your administrative skills and attention to detail will directly support individuals navigating long-term care services.

What You'll Do:




As a key player in our Care Coordination Department, you'll provide essential support to our licensed healthcare practitioners by managing and processing vendor requirements, reviewing documents, tracking recertification processes, and ensuring smooth communication with care providers. This role is crucial in helping our team deliver compassionate, effective care and ensure timely benefit decisions.

Key Responsibilities:




Initiate and manage claims documentation and benefit eligibility requests Review and process key care coordination documents for accuracy Communicate with vendors and internal staff to track and obtain missing information Collaborate with Care Coordinators to ensure enrollees' needs are met and documented accurately Manage recertification tracking and medical record retrievals Support process improvements and special projects

What You Bring:




Strong communication and collaboration skills Ability to prioritize tasks in a fast-paced, multi-tasking environment Proficiency in Microsoft Office Suite Strong technical aptitude and organizational skills A commitment to compliance, accuracy, and customer service

Location:

Portsmouth, NH - Hybrid (2 days per week in the office)


Schedule:

Monday- Friday Typical office hours 8:30am-5pm with opportunity for flexibility. 2 days in office: Tuesdays + 1 day either Monday or Wednesday.


Compensation: $20/hr
