Overview


We are seeking a dedicated and compassionate Mental Health Case Manager to join our team. The ideal candidate will play a crucial role in providing comprehensive support to individuals facing mental health challenges. This position involves working collaboratively with patients, families, and healthcare professionals to ensure effective care coordination and optimal patient outcomes. The Mental Health Case Manager will utilize their expertise in behavioral health and case management to facilitate treatment plans, provide resources, and advocate for patients throughout their care journey.

Duties



Conduct thorough patient assessments to determine mental health needs and develop individualized care plans. Collaborate with interdisciplinary teams, including mental health counselors, psychiatrists, and medical staff, to coordinate patient care. Provide ongoing support and counseling to patients dealing with mental health issues, including addiction counseling and crisis intervention. Monitor patient progress and adjust care plans as necessary based on clinical evaluations and feedback. Facilitate discharge planning for patients transitioning from inpatient to outpatient settings or post-acute care. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Educate patients and families about mental health conditions, treatment options, and available community resources. Conduct intake assessments for new patients entering the program. Participate in utilization management activities to ensure appropriate resource allocation.

Requirements



Experience with Asylum Seekers, Refugees and Immigrants a plus. Strong knowledge of medical terminology, physiology, and behavioral health principles. Proficiency in case management practices including assessments, treatment planning and discharge planning. Understanding of managed care processes and patient service standards. Excellent communication skills with the ability to build rapport with patients from diverse backgrounds. Familiarity with the Mainecare system and the authorization process. Ability to work effectively in a fast-paced environment while managing multiple priorities. Commitment to maintaining confidentiality and adhering to ethical standards in mental health practice. Pay: $20.00 - $35.00 per hour
