Are you passionate about helping others live independently and thrive? At Northeast Michigan Community Mental Health Authority (NeMCMHA), we're looking for dedicated Case Managers to join our mission-driven team and make a meaningful impact across our four-county region.
What You'll Do
As a Case Manager, you'll walk alongside individuals navigating serious mental illness, intellectual/developmental disabilities, autism spectrum disorder (ASD), emotional disturbance, or substance use disorders. You'll help connect them to essential medical, social, educational, and community services--empowering them to live with dignity, independence, and purpose.
Your work will include:
Conducting assessments and developing person-centered service plans
Coordinating care and linking individuals to needed supports
Monitoring progress and adjusting services as needed
Providing crisis intervention and participating in on-call rotation
Collaborating with families, schools, jails, and other agencies
Supporting individuals in the least restrictive, most integrated environments
Compensation
BSW: $23.89 - $30.99/hour (Nonexempt)
RN: $27.72 - $38.27/hour (Exempt)
Full-Time Reports to Case Management Supervisor
Qualifications
Bachelor's degree in Social Work, Education, or Nursing
State registration as an RN or LBSW (or ability to obtain)
At least 1 year of relevant experience in mental health or nursing
Lived experience with mental illness, developmental disabilities, or substance use is valued
Strong communication skills and functional computer literacy (within 6 months of hire)
Valid driver's license and acceptable driving record
