Who are we looking for?
The Youth Outreach Worker engages with and builds relationships with program participants and their families requiring additional support in areas as directed by the program.
What you will do...
Engage and build relationships with program participants and their families needing extra support in areas as directed by the program.
Provide basic case management, follow-up, and support services through weekly or monthly one-on-one meetings with youth.
Identify needs, gaps, and duplicate services; collaboratively find solutions with other agencies to create efficient support services.
Build on existing local resources and communication methods to develop tools aimed at youth and their families.
Engage and strengthen relationships with community partners and other programs/activities the youth participate in.
Participate in community meetings that focus on youth needs and service coordination.
Act as a liaison when making referrals to other agencies that support families in housing, mentoring, mental health care, parenting education, and more.
Update and distribute resource newsletters for families.
May include home visits to students and parents
Assist the attendance department at schools with addressing chronic attendance and engagement concerns.
Other duties as assigned by supervisor.
Education, Experience, and Requirements...
Required:
High School Diploma or equivalent
Valid driver's license with auto liability insurance
Experience working with children, families, and youth
Bilingual - English and Spanish
Successful completion of a comprehensive background check
