Licensed Practical Nurse (LPN) - Skilled Nursing Facility



We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide exceptional care and contribute to a supportive and professional environment.



Responsibilities:


Deliver direct nursing care to residents in accordance with the care plan Administer medications and treatments as prescribed Monitor residents' conditions and report changes to the Registered Nurse or physician Document resident care accurately and timely Supervise Certified Nursing Assistants (CNAs) and ensure care plans are followed Assist with admissions, transfers, and discharges Communicate effectively with residents, families, and interdisciplinary team members Maintain infection control and safety standards in accordance with facility policies
Qualifications:


* Active LPN license
