Violence Outreach Specialist - Muncie - Part Time Days

Overview

Hours: 24 hrs/wk 3-8 hr shifts

This is a grant funded position that has approval for 1 year



Provides support to patients in Delaware County that have been victims of violence. Fosters an environment of trust to encourage positive change following violent incidents. Provides intensive case management and support to patients involved in the program.

Key Responsibilities:

Create and maintain relationships with community partners to expand resource availability for the patients Conducts intake assessments, formulates plans, and monitors patient progress in the program.

Qualifications:

High school diploma, GED, or equivalent. Some higher level training/education preferred. A background and understanding of the population served, having similar lived experience as the clients to be served. Experience with academic institutions and academic medical centers preferred Basic Proficiency in Microsoft Office Knowledge of applicable state and federal laws and regulations


High School Diploma or equivalent required.

0-3 years of experience required. Requires basic computer skills. Requires knowledge of medical, community, and social resources.
