A renowned coordinated care facility is actively seeking a Utilization Management Clinical Support Associate to join their team.

The following qualifications are preferred:

• Associates or bachelor\xe2\x80\x99s in health-related field
• Minimum two (2) years\xe2\x80\x99 experience in a healthcare setting including experience with case management, disease management, and/or quality improvement departments.
• Knowledge of Oregon Medicaid

The Utilization Management Clinical Support Associate will play a vital role within this Coordinated Care Organization (CCO) by providing essential support and assistance to ensure the effective coordination and delivery of healthcare services. They will collaborate with various healthcare professionals, such as physicians, nurses, social workers, and other members of the care team, to develop and manage individualized care plans for patients. By gathering and analyzing patient data and health outcomes, they will evaluate the effectiveness of care delivery and identify areas for improvement. Additionally, they serve as patient advocates, ensuring that patients and their families needs and concerns are addressed by the care team and helping them navigate through the healthcare system.

The Utilization Management Clinical Support Associate will also play a key role in connecting patients with community resources and support services to enhance their overall well-being and aid in their recovery or management of chronic conditions. The Clinical Support Associate will provide patient education on medical conditions, treatment options, and self-management strategies, empowering patients to actively participate in their own care. Through interdisciplinary collaboration, they will facilitate communication and coordination among different specialties involved in a patient\xe2\x80\x99s care. Moreover, the Clinical Support Associate will actively participate in quality improvement initiatives to enhance the overall efficiency and effectiveness of healthcare delivery and achieve better patient outcomes. Ensuring compliance with regulatory requirements and maintaining accurate patient documentation are also important aspects of this role.
