Responsibilities
Manage patient care plans and coordinate services for optimal outcomes.
Conduct assessments to identify patient needs and develop care strategies.
Achieve performance targets and contribute to team success.
Collaborate with healthcare teams to ensure comprehensive patient support.
Participate in ongoing professional development and training opportunities.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Document patient interactions and maintain accurate records.
Utilize data to track performance metrics and improve service delivery.
Engage in regular communication with patients to foster relationships.
Provide feedback to management on service improvements and patient needs.
Stay updated on industry trends and best practices in case management.
Experience/Qualifications
Registered Nurse (RN) license in Tennessee or compact state.
Commitment to ongoing professional development and learning.
Minimum of 2 years experience in case management or similar role.
Excellent communication skills for patient and team interactions.
Ability to engage patients and build trusting relationships.
Strong understanding of patient care plans and healthcare regulations.
Ability to work independently while contributing to team goals.
Strong analytical skills to assess patient needs and outcomes.
Familiarity with performance metrics and achieving targets.
Proficiency in using healthcare software for documentation and tracking.
