Responsibilities
Manage patient care plans and coordinate services for optimal outcomes.
Conduct assessments to identify patient needs and develop care strategies.
Achieve performance targets and contribute to team success.
Collaborate with healthcare teams to ensure comprehensive patient support.
Participate in professional development and training opportunities.
Engage in continuous learning to enhance clinical skills and knowledge.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Document patient interactions and maintain accurate records.
Utilize data to track performance and improve service delivery.
Support patients in navigating healthcare systems and resources.
Provide feedback to improve processes and patient care quality.
Experience/Qualifications
Current California RN license with case management experience preferred.
Demonstrated commitment to continuous professional development and training.
Proven ability to manage and coordinate patient care plans effectively.
Strong assessment skills to identify patient needs and develop strategies.
Experience in collaborating with multidisciplinary healthcare teams.
Ability to monitor patient progress and adjust care plans as needed.
Excellent communication skills for educating patients and families.
Ability to utilize data to track performance and improve services.
Proficient in documenting patient interactions and maintaining records.
Strong problem-solving skills to support patients in healthcare navigation.
