Responsibilities
Manage patient care plans and coordinate services for optimal outcomes.
Conduct assessments to identify patient needs and develop care strategies.
Collaborate with healthcare teams to ensure comprehensive patient support.
Achieve performance targets and contribute to team success.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Document patient interactions and maintain accurate records.
Utilize data to track performance and improve service delivery.
Participate in professional development and training opportunities.
Engage in continuous learning to enhance clinical skills and knowledge.
Participate in quality improvement initiatives to enhance patient care.
Provide support and guidance to junior staff and interns.
Experience/Qualifications
Valid RN license in North Carolina with case management certification preferred.
Commitment to continuous learning and professional development in the healthcare field.
Experience in case management and patient care coordination in a clinical setting.
Proven ability to meet performance targets and contribute to team success.
Strong communication skills to effectively educate patients and collaborate with teams.
Experience in quality improvement initiatives to enhance patient care outcomes.
Ability to thrive in a fast-paced, results-oriented work environment.
Ability to utilize data for performance tracking and care plan adjustments.
Willingness to mentor junior staff and support interns in their development.
Strong organizational skills to manage multiple patient care plans effectively.
