Responsibilities
Manage patient care plans and coordinate services for optimal outcomes.
Conduct assessments to identify patient needs and develop care strategies.
Achieve performance targets and contribute to team success.
Collaborate with healthcare teams to ensure comprehensive patient support.
Participate in ongoing professional development and training opportunities.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Document patient interactions and maintain accurate records.
Utilize data to track performance metrics and improve service delivery.
Implement best practices to enhance patient care and operational efficiency.
Engage in regular communication with patients to foster relationships.
Adapt to changing patient needs and healthcare regulations.
Experience/Qualifications
Valid RN license in Alaska with case management experience.
Proven ability to develop and implement patient care plans.
Commitment to ongoing professional development and training.
Strong communication skills for patient and team interactions.
Ability to adapt to changing patient needs and healthcare regulations.
Ability to engage patients and families in care management.
Experience in using data to track and improve performance metrics.
Experience collaborating with multidisciplinary healthcare teams.
Proficiency in documenting patient interactions accurately.
Strong organizational skills to manage multiple patient cases effectively.
