Responsibilities
Provide high-quality patient care in an ambulatory setting.
Participate in continuous professional development and training.
Achieve performance targets and contribute to team goals.
Collaborate with healthcare teams to optimize patient outcomes.
Educate patients and families on health management and resources.
Engage in self-assessment and seek feedback for improvement.
Monitor and document patient progress and treatment plans.
Utilize data and pipelines to assess patient care effectiveness.
Assist in the development of care plans tailored to individual needs.
Communicate effectively with patients and healthcare professionals.
Maintain compliance with healthcare regulations and standards.
Foster a positive and supportive environment for patients.
Experience/Qualifications
Current Oregon RN license and CPR certification required.
Experience in ambulatory care or outpatient settings preferred.
Commitment to continuous professional development and learning.
Demonstrated ability to achieve performance targets consistently.
Strong communication skills to effectively educate patients and families.
Proven ability to collaborate within multidisciplinary healthcare teams.
Ability to utilize data for assessing patient care effectiveness.
Familiarity with healthcare regulations and compliance standards.
Skills in developing tailored care plans for patients.
Experience in monitoring and documenting patient progress effectively.
