Manager Clinical Resource / Case Management (Travel)

Hyannis, MA.

Key Responsibilities:

Operational & Staff Management

Direct daily operations of Case Management and Social Work teams, including staffing, scheduling, and performance management. Support Clinical Documentation Improvement team functions as needed. Conduct unit rounding to identify workflow barriers and facilitate timely patient discharges.

Education, Training & Professional Development

Design and deliver continuing education programs for Clinical Resource staff. Onboard new team members, including Case Managers, Social Workers, physicians, nurses, and allied health staff, on best practices in discharge planning and utilization review.

Program Monitoring, Compliance & Improvement

Track, analyze, and report program compliance, developing dashboards to identify opportunities for improvement. Lead process improvement initiatives based on performance metrics. Maintain current knowledge of regulatory changes related to case management and utilization review.

Leadership, Collaboration & Continuous Improvement

Mentor and coach clinical teams to enhance collaboration across physicians, nursing, and Clinical Resource members. Promote a culture of continuous improvement by recommending workflow enhancements. Actively support organizational goals and patient care best practices.

Qualifications:

Bachelor's Degree in Nursing required; Master's Degree preferred. Current Massachusetts RN License. Minimum of 5 years of acute-care Case Management experience. At least 2 years of experience in process improvement. Experience developing and presenting continuing education programs. Knowledge of CMS regulations. Strong computer skills, including Windows-based systems. Excellent organizational, analytical, writing, and interpersonal skills.

Preferred Qualifications:

Certification in Case Management or Clinical Documentation Integrity. Prior management or supervisory experience.
