Job Summary
Ignite Hope is seeking a Trauma Case Manager to provide community-based, trauma-informed case management services for clients with behavioral health needs. This position works collaboratively as part of an interdisciplinary treatment team to coordinate care, advocate for clients, and support long-term stability and recovery.
This role is ideal for a mental health professional who is passionate about trauma-informed care, client advocacy, and community engagement.
Key Responsibilities
Conduct comprehensive psychosocial and needs assessments
Participate in the development and monitoring of individualized treatment plans
Coordinate behavioral health, medical, housing, and social service referrals
Monitor client progress and service utilization to ensure continuity of care
Provide crisis intervention support, including follow-up and walk-in assessments
Advocate for clients within community systems and service networks
Maintain accurate and timely clinical documentation, including progress notes, referrals, and discharge planning
Meet productivity expectations and submit billing documentation in compliance with agency standards
Collaborate with internal clinical staff and external community partners
Accompany clients to appointments or community services when necessary
Maintain client charts in accordance with quality, ethical, and regulatory standards
Participate in required staff training and professional development
Required Qualifications
Master's degree in Social Work, Counseling, Psychology, or related behavioral health field
Minimum 2 years of experience in mental health, case management, or behavioral health services
Knowledge of:
Trauma-informed care principles
Mental health counseling and case management practices
DSM-5-TR and diagnostic terminology
Ethical and legal standards in behavioral health
Strong written and verbal communication skills
Ability to work collaboratively with multidisciplinary teams and community agencies
Preferred Qualifications
Experience in community mental health or outpatient settings
Experience working with trauma-impacted populations
Familiarity with Medicaid documentation and billing requirements
