JOB SPECIFICS


The Transportation Navigator provides support to the Mobility Manager and nurtures community relationships to provide transportation resources to residents. The person in this role must be able to work closely and effectively with aging adults, adults with disabilities, and other residents for whom there are barriers to accessing transportation resources. This position requires one-on-one and group interactions with residents (both in-person and virtual) through phone calls, community visits & presentations, and travel training. The Transportation Navigator must be able to organize tasks, communicate effectively, maintain files, work well both independently & on a team, and document activity accurately.


Primary Tasks:


  • Participate in events as both participant and presenter, face-to-face and virtually, to increase the visibility and availability of transportation options and resources.

  • Respond to transportation inquiries received from the website, phone line and email box.

  • Provide travel training with residents in one-on-one and group settings.
    n-person surveys to collect circulator rider survey data (monthly in Stillwater, Forest Lake, and Cottage Grove).

  • Support Mobility Manager during staff & partner trainings, assist with meeting notes and other tasks as directed.

  • Support Volunteer Driver Programs and other local partners as directed.

  • Maintain appropriate documentation.

  • Assist with data collection, data analysis, and grant management.

DESIRABLE EXPERIENCE:

  • Experience interacting with and providing services to individuals from a variety of cultural and ethnic backgrounds

  • Experience (paid, unpaid or volunteer) providing information, outreach, and/or coordination of care to individuals and groups within diverse communities. This includes face-to-face, phone or group interactions, presentations and may include traveling to an individual\xe2\x80\x99s home, community locations, various agencies, and other outreach destinations.
  • Experience (paid, unpaid or volunteer) providing customer service to seniors/aging adults, people with disabilities and/or individuals who have barriers to transportation
  • Experience (paid, unpaid or volunteer) building relationships in a community-based setting with local or community-based organizations (such as schools, community, faith-based organizations, and health care facilities)
  • Experience (paid, unpaid or volunteer) working in or knowledge of public and/or private transportation in the Twin Cities Metro Area and/or surrounding communities



SELECTION TECHNIQUE: Your training and experience will be rated based on the required application material submitted and the top scoring candidates will be forwarded to the hiring department for further consideration.

HOURS / DAYS: To be determined and agreed upon. Includes some early evenings/weekends.

ANTICIPATED HIRING RATE: $24.91 - $29.43/hour

JOB SUMMARY
The Family Service Worker assesses family and home stability, develops, implements and manages case plans and procedures and coordinates appropriate education and support services to ensure compliance with requirements and promote successful, safe and healthy family units.

DUTIES & RESPONSIBILITIES

Develop and implement case plans:
  • Assist in developing, implementing and monitoring case plans with social workers, case managers, parents, family clients or community service providers;
  • Assess family dynamics to determine needs and level of service to be provided;
  • Determine appropriate resources for community based care and services;
  • Make referrals and provide follow-up to ensure services are provided;
  • Assist clients to increase parenting and home or money management skills;
  • Guide client socialization skills development or restoration;
  • Teach clients about health, safety and nutritional needs of their family;
  • Implement plans to address identified child or adult safety issues;
  • Assist in the formation and implementation of parent education or support resources;
  • Coordinate services to assist in client care or further personal development;
  • Facilitate parent education or support group participation;
  • Provide transportation for adults and children as needed;
  • Provide supervised visitation for clients in the child protection system;
  • Assist clients in applying for safe and affordable housing;
  • Conduct in-home assessments of family and home stability for day care licensure and in supervision of court ordered visitations;
  • Document client compliance with case plans and/or court orders;
  • May provide written and oral testimony in court; and
  • Advise clients of rights and responsibilities under programs and services.

Promote community outreach:
  • Collaborate with foster parents, county attorneys, public defenders, health professionals, teachers, service organization personnel and staff from other counties;
  • Develop, coordinate and deliver program information to the general public, special interest groups and professional organizations; and
  • Promote child foster care recruitment.
Perform other duties as assigned.

WORK ENVIRONMENT

Semi-sedentary involving office and outside field work. Lifting requirement may include lifting up to 50 pounds. Exposures may include but not be limited to disagreeable odors, communicable diseases, bodily fluids, and behavior challenges. Use of general office equipment to include telephone, cell phone, pager, personal computer, laptop computer, electronic recorder, calculator, etc. Additional physical/mental requirements that occur may not be listed above, but are inherent in performing the position\'s essential functions.
