Responsibilities/:

The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.

Responsibilities

Coordinate patient care in both pre- and post-transplant phases Collaborate with interdisciplinary team members within the organization and outside community partners Act as a liaison for transplant physicians to implement medical management of conditions Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes Apply appropriate medical interventions and transplant protocols based on patient assessment, laboratory results, and patient condition changes Assumes on-call responsibilities after office hours by providing a resource for medical triage. Acts as transplant coordinator when donor organ becomes available. Provides on-going education to patients and families Collaborate with other members of the clinical team to improve patient care and benchmarks through quality improvement projects Participate in the development and revision of protocols Maintain knowledge of current practices in transplantation, nursing, and healthcare

Required Qualifications

Bachelors Degree in Nursing or Associates Degree in Nursing If Bachelors Degree: 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg or If Associates Degree: 5+ years of acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg MN Registered Nurse (RN) License Basic Life Support (American Heart Assoc or Red Cross)
