Job Description

PURPOSE OF POSITION



The Case Manager position is responsible for overseeing, supporting, and coordinating services for participants who

reside in independent and community housing. The Case Manager will work closely with participants to achieve the

goals of their individual case management plan.

SUMMARY OF DUTIES

(Responsibilities to include but not limited to the following)

Overarching Responsibilities:

? Completes intake packet and release of information forms with each new participant.

? Conducts risk and needs assessments with each participant to identify areas of need.

? Develops case plans in conjunction with the participant and supportive services team.

? Oversees the implementation of the case management plan.

? Monitors participant participation and progress with community referrals by establishing and maintaining

regular communication with community partners.

? Interacts with participants individually and in groups to offer them supportive assistance and programming.

Coordinates services with other staff members.

? Coordinates with outside agencies to help participants utilize needed services. Communicates regularly with

community partners and seeks new partners in the community.

? Participates in staff meetings and other meetings to monitor and promote participants' success in the

program.

? The case manager works closely with the Program Services and Operations Teams to ensure successful

delivery of services.

? The case manager maintains connections with community service providers and links participants to those

resources.

? Records, documents, and tracks each participant's activities and progress towards goals. Reviews and/or

completes other paperwork as needed.

? Participates in training and staff development opportunities.

? Act as member of the on-call team and rotation.

? Performs other duties as assigned.

SUPERVISORY DUTIES

This position has no supervisory responsibilities.

KNOWLEDGE, SKILLS, and ABILITIES:

Education:

? Bachelor's degree in social work, occupational therapy, psychology, or closely related field or

equivalent experience preferred.

Experience:

? Minimum of two years of work experience working with justice-involved females, women with substance

use histories, and/or other at-risk populations.

Additional Requirements

? Excellent communication skills for engaging with diverse populations, including participants and team

members.

? Proficient in basic computer usage, including familiarity with Microsoft office.

? Ability to work collaboratively and independently.

? Ability to accompany and transport participants to critical appointments.

? Possess a valid Class E driver's license or be willing to obtain one within 1 month of hire.

? Have a safe driving record and ability to operate a vehicle to transport participants as required.

? Provide a Motor Vehicle Record and valid car insurance for personal vehicle yearly or as needed.

WORKING RELATIONSHIPS

? Frequent and regular contact with Program Services and Operations Team.

? Works closely with Department of Corrections staff.

? Works with community service providers linking participants to resources.

? Regular contact with program participants.

? Regular contact with the Community Learning and Engagement Specialist.

? Regular contact with Peer Support Specialist, Transition Support Specialist, Vocational Specialist and House

Monitor Team.

KNOWLEDGE, SKILLS, and ABILITIES:

Work Environment/Physical Demands:

? Adaptable to a dynamic work environment while prioritizing personal and team well-being.

? Potential for moderate eye strain due to documentation and computer work.

? Ability to demonstrate good finger/hand dexterity for typing, sorting, filing etc.

? Ability to demonstrate the physical stamina to do some walking and stair climbing; moderate pushing,

pulling, kneeling, stooping, bending, and lifting.

? Ability to work under stress.

? Ability to work some evenings and weekends

Pay: $44,000.00 - $48,000.00 per year
