Job Description


The Department of Mental Health (DMH) is seeking a Transition Case Management Supervisor to provide direct supervision to Transition Case Managers who provide DMH transition case management services to individuals ages 22 and older. Transition Case Managers carry a caseload of assigned clients, and their duties consist of active engagement, assessment of service needs, and service planning activities to facilitate the discharge or diversion of individuals from nursing facilities.

Responsibilities include implementation of decisions made through the Preadmission Screening and Resident Review (PASRR) process; assignment of clients to transition case managers; and ensuring that all transition case management services comply with all DMH and PASRR regulations and procedures.

Transition Case Management Supervisor will serve as the DMH community liaison to nursing facilities; hospitals; care management/coordination resources including but not limited to Behavioral Health Community Partners (BH CPs), Community Transition Liaisons (CTLs), One Care plans and Senior Care Organizations (SCOs); DMH contracted and non-contracted community programs; and other state agencies.

Duties and Responsibilities (these duties are a general summary and not all inclusive):

  • Receive clinical eligibility determination from Nurse Consultant.
  • Assign Transition Case Manager, authorize services as appropriate, enter information into MHIS.
  • Provide weekly supervision to Transition Case Managers and maintain documentation of meetings.
  • Review transition case management activities including but not limited to face-to-face contacts, comprehensive assessments, and Individual Service Plans (ISPs). Monitor client goal attainment through case management supervision and, as necessary, reviews of discharge/transition plans.
  • Respond to instances of concern regarding services provided to individual clients and assure successful resolution.
  • Maintain relevant wait lists for community services.
  • Complete EPRS for all case managers.
  • Authorize use of benefit time and maintain adequate coverage.
  • Provide relevant information to Nursing Facility Transition Manager and Nursing Facility Transition Specialist on programmatic issues that impact service delivery.
  • Participate with Nursing Facility Transition Manager and Nursing Facility Transition Specialist in planning and development of transition case management practices and procedures, and development of new services, as needed.
  • Identify service gaps, strengths, and limitations
  • Participate with Nursing Facility Transition Manager and Nursing Facility Transition Specialist in clinical review meetings to identify barriers to discharge and/or diversion from nursing facilities
Required Qualifications:
  • Demonstrated knowledge of psychosocial problems of those with mental/emotional disorders and the types and symptoms
  • Knowledge of methods of general report writing with the ability to assess case management process and results.
  • Ability to effectively organize and run meetings and case conferences to promote collaboration and information sharing.
  • Ability to understand, support and implement the principles of race, equity, and inclusion and work cooperatively and effectively with co-workers, clients, provider staff and community partners.
  • Lead and supervise a team and the ability to give oral and written instructions in a clear and precise manner.
  • Knowledge of principles and practices of crisis intervention.
  • Ability to prioritize and multitask duties based on client need, team, and service needs.
  • Knowledge of the special needs of individuals currently residing in nursing facilities who may be able to be discharged or diverted from nursing facilities.
Preferred Qualifications
  • Familiar with the types of community programs and services available to individuals living with mental illness.
  • Familiarity with nursing facilities and community services for older adults and adults with disabilities.
  • Knowledge of the Preadmission Screening and Resident Review (PASRR) regulations and procedures.
  • Knowledge of adult psychiatric medications and community-based resources appropriate for transition case managed clients.
  • Given the population served, bilingual or multilingual fluency

MINIMUM ENTRANCE REQUIREMENTS:

Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator, within the Department of Mental Health, a Bachelor\'s degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required.

In addition applicants must have at least four (4) years of full-time, or equivalent part-time, case management of clinical/therapeutic experience.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver\'s license at a class level specific to assignment.
