Job Summary




Employee is responsible for thoroughly analyzing the medical record, extracting, validating, and coding diagnoses and procedures according to established regulatory guidelines; conferring with the provider as needed to obtain the correct reimbursement in a timely manner for all encounters. Responsible for interpreting, translating, editing, and transcribing dictation for content and clarity with timelines and accuracy; assisting providers and other MCH staff with dictation and transcription as needed. Duties also include retrieving and releasing medical records according to Federal and State laws.

Essential Functions




Assigns appropriate ICD-9 and CPT codes, as well as necessary modifiers to all relevant diagnoses and procedures in accordance with accepted coding principals, standards, departmental guidelines, and in compliance with regulatory agencies.

Communicates as needed with Patient Accounts for clarification of coding/billing issues.

Remains knowledgeable of Coding Guidelines, CPT Assistant guidelines, and coding/regulatory changes by reading literature, trade journals, etc. and attending conferences/workshops.

Transcribes all work types and all physician specialty dictation with accuracy and efficiency and with minimal assistance.

Investigates unclear or potentially inaccurate dictation or demographic information to assure accuracy of transcribed reports for patient safety.

Maintains at least the minimum quality and productivity standards on a consistent basis.

Releases protected health information in accordance with hospital policy and HIPAA laws.

Responds to requests for patient information via fax, phone, or mail.

Answers the telephone and assists walk-in customers.

Scans medical records into Electronic Medical Record accurately.

Files completed medical records accurately and efficiently where appropriate.

Uses chart location/tracking to assure records are accounted for at all times.

Demonstrates and actively promotes an understanding and commitment to the mission of MCH.

Qualifications




Must be a Registered Health Information Administrator (RHIA), a Registered Health Information Tech (RHIT), Certified Professional Coder (CPC), a Certified Coding Specialist (CCS) or a graduate of a Health Information Management Administration or Technician Program or AHIMA Coding Basics Program, thus making the individual eligible to take the AHIMA or AAPC registration/certification examination. Registration/certification must be achieved within 12 months of hire.

Graduate of a 1 or 2 year medical transcriptionist program, Health Information Technician program or a minimum of 1-2 years current medical transcription experience, preferably in a hospital setting.

Must be very detail-oriented and possess the ability to analyze, interpret, and extract detailed technical clinical data for long periods of time.

Must have excellent communication skills to interact with physicians and staff from various departments and levels in the organization.

Must be able to use independent judgment in assessing documentation and applying coding guidelines.

Must have demonstrated competence in the use of personal computers and email.
