Key Responsibilities

Deliver comprehensive virtual primary care services via video and phone consultations, including routine health maintenance, chronic disease management, preventive care, and acute issue management. Conduct remote assessments, order and interpret diagnostic tests, and prescribe medications as clinically appropriate under applicable telehealth and state regulations. Develop and manage individualized care plans in collaboration with patients, families, and interdisciplinary care teams. Coordinate referrals, follow-ups, and continuity of care with specialists, pharmacies, and community resources. Provide education and counseling to patients on preventive care, treatment adherence, and self-management strategies for chronic conditions. Maintain accurate and timely documentation of all patient encounters in the electronic health record (EHR). Ensure compliance with all telehealth clinical standards, privacy laws (HIPAA), and organizational protocols.

Qualifications and Job Requirements

Active Nurse Practitioner License in Michigan (required; multi-state licensure a plus). Master's Degree in Nursing or higher (required). 2+ years of experience in primary care, family medicine, internal medicine, or urgent care preferred. Demonstrated ability to provide safe, effective, and empathetic care in a virtual environment. Strong clinical assessment, diagnostic, and communication skills suited for remote patient interactions. Comfortable with EHR platforms, telehealth technologies, and digital communication tools. Organized and self-motivated, with excellent time management skills and a commitment to patient-centered care. Prior telehealth experience is highly desirable.
