Job Description

The Atawisamatas Home Visitor (AHV) will be responsible for providing direct outreach to the CTUIR community, client family participants, providers, departments, and organizations involved in direct services, as well as community leaders and advocates. The AHV will provide home visits to clients/families using Johns Hopkins University's Family Spirit evidence-based curriculum. The Tribal Maternal, Infant, and Early Childhood Home Visiting (TMIECHV) grant, through the Administration for Children and Families, funds this program through October 1, 2028. The purpose of this program is to promote improved health, development, and emotional outcomes in maternal, infant, child, and family populations served at Yellowhawk. As an Atawisamatas Home Visitor, the position will promote healthy pregnancies, breastfeeding, and parent-infant attachment. While establishing a trusting relationship with mothers and caregivers, the AHV will provide prenatal, labor, and postpartum support. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statement.


Essential Job Functions:





1. Work under the direction of the Atawisamatas Program Manager to provide home visiting services to families and various settings.


2. Work collaboratively with Program Manager and other care teams to coordinate care, advocate patient needs, and develop comprehensive care plans.



3. Ability to present, communicate, and teach to small and large groups in a clear, engaging manner.



4. Perform home visits to assess patient and family healthcare and social needs to promote positive parenting and healthy living.



5. Understand grant objectives, strategies, and manage collection and maintenance of grant data and reporting requirements.



6. Complete Community Health Worker Certification and be active in adherence to policies, procedures and program strategies.



7. Maintain certifications in Family Spirit Curriculum and Fatherhood is Sacred/Motherhood is Sacred and follow Yellowhawk Home Visiting Policy and Procedures.



8. Collaborate with other departments to maintain communication and good working relationships within the organization to ensure quality health care service for patients.



9. Collaborates with Yellowhawk Public Relations Department for the creation of social media campaigns, advertising, and all print materials utilizing the Communications Policies in place.



10. Provide service link and referral resources to assist families with their healthcare and social needs.



11. Represents Yellowhawk with a respected and viable presence at community events, within the clinic, and while in attendance at conferences, courses, and events outside of the CTUIR boundaries.



12. Maintains confidentiality in accordance with the Yellowhawk Tribal Health Center Policy Manual, HIPAA and the Privacy Act.



13. Other duties as assigned.

Minimum Requirements:





1. Associate's Degree in Social Work, Early Childhood Education, Psychology or related field



OR



High School Diploma and 2 years relevant work experience with youth or families in Tribal communities.



2. General understanding of trauma-informed approaches to working with women, children and families.



3. Working knowledge of Microsoft Office programs and/or other relevant software to meet the needs of various standards in both clinical and grant-related documentation.



4. Must possess high-level verbal and written communication skills.



5. Demonstrated ability to work effectively in a team setting with diverse membership.



6. Must complete Community Health Worker within 18 months of hire (Dependent upon training availability).



7. Must possess or receive CPR and First Aid certification within ninety (90) days of employment.



8. Must possess a valid driver's license and maintain insurance requirements to operate General Services Administration vehicles.



9. Must submit to and pass a reference and criminal background check.



10. Must submit to and pass a pre-employment drug and alcohol screening.


Preferred Qualifications:





1. 1. Bachelor's Degree in Social Work, Early Childhood Education, Public Health, or related field.
