Job Description

This application is for in person with some telehealth Targeted Case Managment services

Responsibilities:

- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop appropriate service plans
- Coordinate and monitor the delivery of services to ensure client needs are met
- Collaborate with other professionals and agencies to provide comprehensive support
- Advocate for clients' rights and access to resources
- Assist clients in accessing childcare, workforce development programs, addiction counseling, child welfare services, Medicare, and other necessary resources
- Provide crisis management and intervention as needed
- Offer mentoring and support to clients to promote personal growth and self-sufficiency
- Utilize motivational interviewing techniques to engage clients in the change process
- Maintain accurate and up-to-date case records

Qualifications

:
- Bachelor's degree in social work, psychology, or a related field

-Current TCM Certification and training in SMI, SED, and/or CCPHC
- Experience in case management or a related field (minimum 1 year)
- Knowledge of behavioral health issues and interventions
- Familiarity with community resources and support services
- Strong communication and interpersonal skills
- Ability to work effectively with diverse populations
- Excellent organizational and time management skills
- Ability to handle crisis situations calmly and effectively

Job Type: Part-time

Pay: $25.00 - $40.00 per hour
